Rates

The fee for an initial consultation and records review is $400, then a $200 hourly rate is applied. We offer a sliding scale for the subsequent hourly rate. Please contact us and provide your household annual gross income for more information.

Direct-pay practice. We do not bill insurance directly, but we provide patients with Health Insurance Claim Forms (HICF).

1. Submit your HICF. Most insurance providers have an online form and/or mailing address to submit the HICF. Be sure to submit by the deadline set by your insurance provider (often 90 days after the date of service).
2. Receive your EOB. Your insurance provider will determine coverage, which will be detailed in an Explanation of Benefits (EOB) sent to you.

Coverage is at the sole discretion of your insurance provider and depends on your plan, deductible, and out-of-pocket maximum. Dr. Jamison’s services are typically reimbursed at the PPO out-of-network rate (some covering 20-60% of cost after your deductible). We do not contract with HMOs, Medicare, or MediCal.
